Alex's Lemonade Stand Foundation is happy to provide a free treatment journal to families of childhood cancer. This journal, which comes in both English and Spanish translations, will help caregivers stay organized by keeping track of important information such as:
- Medications
- Treatment expenses
- Symptoms and side effects
- Complete Blood Counts
- Scan/Diagnostic tests
- Transfusions
- Appointments
- Out-of-Pocket expenses
- Nutrition
- ..and much more!
Please note our treatment journals are intended for families of children with cancer only. Caregivers may request a hard copy, and social workers may request a bulk amount to distribute to their families.
For Healthcare Professionals:
Requests for ALSF Bulk Materials must be submitted through Proposal Central. This allows you to track submitted requests, update your contact information, and communicate directly with program staff.
